Elder Family Care and Kinship Care

If you have any questions about your eligibility for Elder Family Care or Kinship Care which relate to your eligibility for Medicaid and the Choices for Independence (CFI) program, just call us at (603) 206-2725. We’re here to help guide you through the process. Our Elder Family Care  program is also available on a private pay basis.

In general, candidates who qualify for funding for Elder Family Care and Kinship Care:

  • Meet both financial and clinical eligibility requirements for New Hampshire Bureau of Elderly and Adult Services (BEAS’) Choices for Independence (CFI) Program
  • Are declining and may be losing the ability to live alone
  • Express an interest in living with, sharing a home with and receiving services from an individual, couple or family
  • May already be living with a family caregiver and are eligible for the CFI program due to increasing care needs
  • May be planning to leave a nursing facility but do not want or are unable to live alone or with their own family members
  • May be seeking housing in a transitional setting while awaiting the availability of other permanent housing
  • Are able to use the telephone effectively during an emergency
  • Are not in need of continuous skilled nursing care except during periods of acute illness
  • Can be left alone and unsupervised for a portion of the day
  • Are not a danger to themselves or others
  • Have medical diagnoses and conditions that can be safely managed in a private home setting

Elder Family Care and Kinship Care are not structured to serve individuals in need of emergency placement.

In-home Personal Care

Pay Privately

If you choose to pay for Personal Care Services directly simply call us and we can start the process. We’ll work with you to tailor a care plan to your needs and we can discuss our fees, which are some of the most competitive available.

Medicaid Members

We also accept Medicaid and can help you determine if you meet the medical and income requirements. If you’ve already qualified for Medicaid, we can work together to put your customized care plan in place.
