Kinship Care & Adult Family Care

feature_member_of_the_familyKinship Care and Adult Family Care are two options that provide care in a family setting for those who can no longer live safely and independently in their own home.

1. Kinship Care

The Moore Center’s Kinship Care program enables families to be compensated for caring for a loved one at home, in lieu of having them placed in a nursing home. To participate in Kinship Care an individual must be qualified for Medicaid and the Choices for Independence program through ServiceLink. You can visit their website or call them at 866-634-9412. Here are the steps to participate in our Kinship Care program:

  1. Get qualified for Medicaid and the Choices for Independence program
  2. Once you’re qualified they’ll assign you to a case manager
  3. Have your case manager contact The Moore Center to enroll in the Kinship Care program

2. Adult Family Care

Under Adult Family Care a senior can live with an unrelated host family instead of being placed in a nursing home. The “host family” is paid through Medicaid to provide the care. For Adult Family Care to be covered through Medicaid, Choices for Independence qualification is also a requirement. Under Adult Family Care a provider family will welcome a senior into their home and make them part of their family. All provider families are certified by the State of New Hampshire prior to any placement and provide a supportive and loving environment to ensure a positive and meaningful quality of life. 

Seniors have a private room, with access to the rest of the home, and families provide services such as meals, laundry, medication management, appointment coordination, transportation and shopping, as well as other non-medical support based on needs. Moore Options for Seniors provides regular oversight and visits to ensure that the needs of all those involved are being met and that the experience is a mutually beneficial one.

You Can Become a Host family – Our goal is to ensure that every Adult Family Care placement is truly mutually enriching. If you’re interested in becoming a host family you’ll be compensated for your services, and it will also be an experience that enriches your life while making all the difference in the world to the senior who moves into your home.  Learn more about becoming a host family.

Adult Family Care – Private Pay

Adult Family Care is also available as a private pay program for families who can’t care for elderly loved ones themselves but prefer a family environment instead of a nursing home. You can place your family member with a host family who will provide the care needed, while The Moore Center provides oversight to ensure that the placement works to everyone’s satisfaction. Too many elderly end up in a nursing home that:

  • Reduces quality of life

  • Takes away choice & dignity

We provide oversight and are available as a 7×24 resource, and we also provide monthly visits to make sure the living experience is a positive one. Our provider families have clear expectations and are committed to enabling the fullest life possible, including continued participation in activities, hobbies, visits with family, shopping and other outings.

Secure

Moore Options for Seniors recruits, screens and trains all provider families. All homes are certified through the Bureau of Health Facilities Administration (Using standards under Rule He-P 813), and all provider families have a criminal, motor vehicle and BEAS background check, as well as a life safety inspection from their local fire department. If at any time an individual doesn’t feel safe, secure or comfortable in their environment, they can call Moore Options for Seniors and we’ll be there.
