Kinship Care & Adult Family Care

Kinship Care and Adult Family Care are two options that enable loved ones to enjoy life in a family home instead of a nursing home. They provide care in a family setting for those who can no longer live safely and independently in their own home. 

Kinship Care

The Moore Center’s Kinship Care program enables families to be compensated for caring for a loved one at home, in lieu of having them placed in a nursing home.

To participate in Kinship Care an individual must be qualified for Medicaid and the Choices for Independence program through ServiceLink. You can visit their website here or call them at 866-634-9412. You can also download their brochure for more information and a list of local offices.

Here are the steps to participate in our Kinship Care program:

  1. Get qualified for Medicaid and the Choices for Independence program
  2. Once you’re qualified they’ll assign you to a case manager
  3. Have your case manager contact The Moore Center to enroll in the Kinship Care program

Adult Family Care

Under Adult Family Care a senior can live with an unrelated host family instead of being placed in a nursing home. The “host family” is paid through Medicaid to provide the care. For Adult Family Care to be covered through Medicaid, Choices for Independence qualification is also a requirement. Under Adult Family Care a provider family will welcome a senior into their home and make them part of their family. All provider families are certified by the State of New Hampshire prior to any placement and provide a supportive and loving environment to ensure a positive and meaningful quality of life. 

Seniors have a private room, with access to the rest of the home, and families provide services such as meals, laundry, medication management, appointment coordination, transportation and shopping, as well as other non-medical support based on needs. Moore Options for Seniors provides regular oversight and visits to ensure that the needs of all those involved are being met and that the experience is a mutually beneficial one.
