How do you Qualify?

To start the process and determine if you are eligible for Kinship Care, the first step is to contact a  ServiceLink office near you. They will determine if you are eligible for Medicaid and the Choices for Independence (CFI) program. They determine this based on care needs and financial assets of the person in need of care.

Once eligibility is determined, you will be assigned to a case manager and we will get notification and authorization directly from them. Once we have that in hand, our Program Coordinator at Moore Options for Seniors will give you a call to set up your intake meeting to get you started.
